To convert physical shares into demat form:

1. Download the DRF form below, ensure it is accurately filled, and duly signed.
2. Attach the below documents:
   • Dematerialisation Request Form (DRF)/Letter of Confirmation(LOC)
   • Self-attested copy of PAN Card
   • Original share certificates
   • Address proof documents (If your address has changed)
3. Courier it to the below address:
